Electronic Communication Terms of Agreement

At Millhurst Medical Clinic, we offer the option to receive appointment notifications via email or text message. These notifications are for administrative purposes only, and no sensitive medical information or personal health data will be shared electronically. All notifications will be sent from a “Do Not Reply” email or text address. Patients must not attempt to reply or send messages to their physician through these channels.


Patient Acknowledgment and Agreement

By consenting to receive electronic appointment notifications, you acknowledge and agree to the following:

  • I have read and understand the risks, limitations, and conditions associated with receiving appointment notifications via email or text, as outlined in this agreement.

  • I understand that no medical advice, health information, or personal data will be shared via email or text.

  • I acknowledge that email and text communications from Millhurst Medical Clinic may not be encrypted, and I accept the associated privacy risks.

  • I agree that either I or my physician may choose to discontinue electronic communication at any time by providing written notice.

  • I understand that any medical questions, urgent concerns, or health-related matters must be directed to the clinic by phone or addressed in person—not through email or text messages.


Risks of Electronic Communication

While Millhurst Medical Clinic takes reasonable steps to protect your privacy, the following risks are inherent in using email or text for communication:

  • Emails or texts may be intercepted, forwarded, altered, or misdirected without your or our knowledge.

  • Backup copies may exist even after deletion.

  • Communications may be accessed by employers, service providers, or third parties.

  • Email or text communications can introduce viruses or malware to your device or network.

  • There is no guaranteed method to verify the identity of the sender or recipient once a message is sent.

  • Court orders or legal obligations may require disclosure of communication records.

  • Messages may be misdelivered or received by unintended recipients.


Conditions for Use of Email/Text Notification Services

  • These services are used strictly for appointment notifications and not for:

    • Discussing medical conditions

    • Requesting prescription refills

    • Sharing lab results

    • Urgent or emergency care

  • The patient is responsible for:

    • Not using work or third-party devices to receive messages.

    • Keeping their contact information updated with the clinic.

    • Protecting the privacy of their devices and inboxes (e.g., use passwords and privacy settings).

  • The clinic is not responsible for issues arising from:

    • Software or device malfunctions

    • Internet service provider failures

    • Unauthorized access due to shared or unsecured devices


Important Contact Instructions

Millhurst Medical Clinic will not monitor or respond to replies sent to appointment notifications.

For any medical concerns or urgent health issues:

  • Call our clinic directly

  • Dial 911 in an emergency

  • Visit your nearest Emergency Department or Urgent Care Centre

  • Call Health Link at 811 for 24/7 non-emergency health advice


Consent Withdrawal

You may withdraw your consent to receive electronic communications at any time by providing written notice to Millhurst Medical Clinic.
